Treatment residents complete an after-care assessment as part of the after-care group while they are still a primary-care resident.
The assessment highlights what areas of their life they have seen growth in and also encourages goal setting. Goals are then used to provide recommendations for their after-care. This process is strength-based and client-centreed, meaning residents look at their strengths and sucesses in treatment.
We recommend that participants look at an area that they have not yet been able to complete while in primary-care. We have found that goals examined from a client-centered view add personal commitment to their goal and increases the likelihood of completion for the goal set.
