Transitional Housing & Comprehensive Aftercare Plans

Veterans who are nearing the end of their stay work individually with Resident Reporting Counsellor Matt Rands (CCAC) to create a comprehensive and personalized aftercare plan. Each individual plan provides a series of specific requirements tailored to ensure ongoing success and growth in recovery.

TWC’s aftercare plans outline necessities such as meeting attendance, further one-on-one counselling, maintenance of healthy relationships, identifying triggers, healthy coping mechanisms, daily recovery maintenance, emergency contacts and a weekly schedule completed by the Veteran to ensure structure is maintained. This aftercare plan can be amended and reconfigured as residents enjoy successes and face new challenges in early recovery.

Transitional homes are dispersed throughout Greater Vancouver and Victoria, serving as a launching pad for men to explore work, volunteering, and educational opportunities. It offers a safe living environment with a level of stability and structure—increasing the odds of maintaining long-term sobriety.

All of our homes are occupied by a live-in house manager who ensures that residents are adhering to rules and following their individualized aftercare plans. Completing treatment and returning to work or school is often challenging. Our transitional housing program offers a safe, community-based setting for men to begin the next phase of their recovery.

Resident Reporting Counsellor

Matt has been with TWC since 2014. Matt specializes in one-on-one counselling for primary residential treatment clients and is proficient regarding issues such as trauma, self-worth, and grief. He has also attained a designation as a Canadian Certified Addiction Counsellor (CCAC) from the CACCF.

Using his twenty-four years’ experience with addiction and recovery, Matt took this knowledge and has applied it at VCC’s Substance Abuse Counselling Program.
